REACH (Racial and Ethnic Approaches to Community Health) is a national program administered by the Centers for Disease Control and Prevention (CDC) to reduce racial and ethnic health disparities. In 2018, the Penn State College of Medicine was recipient of one of the 5-year REACH awards. In full coordination with our community partners, including Better Together of Lebanon, we will use community driven, evidence based, and culturally tailored interventions to improve nutrition, physical activity and clinical linkages among Hispanics/Latinos in Lebanon and Berks counties. Our team will work with local agencies and community organizations to carry out the activities of this project.
